Appointment Checklist - What to Bring



Proof of Social Security Number (i.e. Social Security Card, W2, Pay Stub)



Acceptable forms of identification (must include a photo ID & must bring original copies of documents)

•    Social Security Card
•    Driver’s License
•    ID Card Issued by a Government Agency or other entity (e.g. Military ID Card, School, Employment, State, etc.)
•    Voter Registration Card
•    U.S. Military Draft Record
•    U.S. Military Discharge Form (DOD 214)
•    Native American Tribal Card
•    Valid Passport
•    Bank Statement
•    Government Assistance Programs
•    Permanent Residence Card or any document issued by CIS
•    Laser Visa
•    Border Crossing Card
•    Health Insurance Card
•    Adoption Record
•    Marriage / Divorce Record
•    W-2 Form or Pay Stub
•    Certified Birth Certificate
•    Hunting / Fishing License

Proof of current local address (i.e. mail addressed to your name from the last two months, electronic version of a current bill, bank statement, utility bill)



List of current prescription medications + dosages, as well as start date (if recent)



List of immunizations received within the last 12 months & for any tattoos and/or piercing in the past 12 months, we will ask the date and place of business

Question: How much water should I drink before & after donation?

Answer: 6-8 cups

Question: Why does BioLife use fingerprint, and what do we do with the fingerprint and personal information?

Answer: BioLife uses finger scanning technology to facilitate safe, secure, and efficient identification prior to and during an individual’s donation The finger scan does not record your actual fingerprint. It creates and stores a numerical representation of it within a closed environment. Not only is representation highly encrypted during capture and storage, but not enough data is collected to recreate a fingerprint. Although extremely accurate in configuration, it maintains privacy and is confidential.
